Terms and conditions for 5x/Month Membership

FIT4MOM MONTHLY MEMBERSHIP AGREEMENT

This contract is made between the client and FIT4MOM of Arlington/Mansfield/Midlothian. This is a month to month agreement and may be terminated by either party with the proper advance notice. There is a minimum 3 month commitment on promotions. This membership allows you to attend five Stroller Strides/Stroller Barre/Strides 360 class(es) per month.

AUTHORIZATION FOR PAYMENT
I authorize FIT4MOM Arlington/Mansfield to initiate a charge to the account entered into Front Desk for my dues. This authorization is to remain effective until I have cancelled my membership through the FIT4MOM Arlington/Mansfield administrator.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Arlington/Mansfield to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the first day of the month following purchase of this membership. If charges are returned they will be subject to a $10 late fee. Members are responsible for updating their credit card information online. Members with unpaid balances may not attend class. If a membership remains unpaid for more than 30 days it will be cancelled. If the member wishes to re-enroll they will be charged half the normal registration fee.

HOLD POLICY
To place a hold on this membership, the member must contact the FIT4MOM Arlington/Mansfield administrator via email (amylloyd@fit4mom.com) a minimum of 14 days prior to their next payment due date to request a hold. The member can expect a response from the administrator within 24 hours of request. No hold will be considered valid without administrator confirmation. Memberships may be placed on hold a maximum of 4 months per year (12 month period), and a maximum of 3 months per hold period. Holds exceeding 4 months per year will inactivate this membership. If the member wishes to re-enroll they will be charged half the normal registration fee. Hold starts will coincide with the next date dues are scheduled to be drawn out on and can end on any day of the month. When a hold period is ended, the monthly auto payments will resume on the return date. Prorated fees may be applied if return happens before scheduled billing date. There will be a $15 processing fee charged to your credit card on file per hold period.

CANCELLATION POLICY
To cancel this membership, the member must contact the FIT4MOM Arlington/Mansfield administrator via email (amylloyd@fit4mom.com) a minimum of 30 days prior their next payment due date to request cancellation. The member can expect a response from the administrator within 24 hours of request. No cancellation will be considered valid without administrator confirmation. No refunds will be given for dues once they have been paid. This membership is not transferable to another person or to any franchise outside of FIT4MOM Arlington/Mansfield. Once a membership is cancelled, if the member wishes to re-enroll they will be charged half the normal registration fee.
